Problem 2) is that the baby has a persistent bilateral ear infection
that has not responded to three different meds, amoxicillin (of
course), bactrim, and cephalexin.

I was wondering about the sleep patterns of the baby. I find in
babies that have multiple ear infections a tendency to feed often
during the night. I think these babies get a major portion of their
feeding lying flat on their backs.

Perhaps also related is a tendency in these babies to be highly
sensitive to mother's intake of dairy products.

Let us know.
