There is an appendix in "Milk, Money & Madness: the culture and politics of
breastfeeding" that lists formula recalls from 1982 - 1994.  It is the same
type of list (and inspired by) as Marsha Walker, but a bit more current.
Also, Marsha recently posted information about the FDA web site where you
