Little or no data exist on triamterene concentrations in breastmilk.  I would
assume that they are somewhat low,  but I do not really know.

Traimterene is a potassium-sparing diuretic and is sometimes added to
hydrochlorthiazide and other thiazide diuretics to reduce the potassium loss.
I seriously doubt the antihypertensive effect is that much more or that adding
triamterene would greatly potentiate the antihypertensive effect of a thiazide.

Sorry we know so little about this product.  Observe for elevated potassium
levels.  In this case,  I doubt that adding triamterene would significanly
lower this patients blood pressure.
