> 812-475-0123I have a LC consultation Report form that I use as a chart drop in for in-house moms.  I
also use the same form for private consults and am getting ready to make changes for
phone use.  I write my info down after the phone call in order to concentrate better
when talking.  This will not work for you unless you remeber details well.  I can send
you a copy of the dropin and you may use it as long as you site the source of it.  All
staff reads the drop in so it is really a great piece of paper to have.  Most RNs will
include it with chart if you requests it.  Be sure to xerox it before you leave hospital
so you have a copy to write reports from.  Hospital has given me access to their copier
for this reason so you might do well to ask.
