Chao-Huei,
I saw a mom who was positioning her baby too high when she nursed lying
on her side. The baby used his tongue to try to lift the nipple into his
mouth, causing a crack at the base of the nipple. Moving him downward
allowed him to latch on normally. A mother with a skin condition like
eczema or thrush would be more likely to get cracks, because the skin is
'friable'. Careful positioning and latch on could help avoid further
damage. If this mother has access to LLL, maybe they could show her good
positioning.
