I just received this in the mail and wondered if anyone else has seen the
same.  Here is a portion of the cover letter.

Dear Expectant Mother:
This is an invitation to become a member of a very select group, The National
Opinion Network (Infant Division).  As a member, you will participate in
marketing reserch studies that will help manufacturers throughout the world
in their efforts to design and develop new products, and improve today's
products. (Blah Blah Blah)

The questionnaire starts out with five questions about prenatal care
pratices.  The following 11 are about feeding.  The following state they are
for classification purposes.  They ask about government participation (WIC
etc.)  The number of children.  What feeding method was used with the last
child and what formula was used.  This is followed by questions regarding
employment, education, birthdate, race, and income.

The name at the bottom of the letter is Kathy Wilson (Project Director).
