Re:     Accutane

        For some very "specific"  reasons,  I didn't put "Accutane" in my
book,  but I did put the generic name isotretinoin (hell, I just forgot it).
Accutane is a vitamin A derivative,  that is absolutely,  horribly
teratogenic.  It is extremely lipid soluble, and accumulates in the liver in
large concentrations,  and in some patients, can produce signficant liver
toxicity.  For cystic acne,  it works fabulously,  as long as you know the
side effects and can watch the patient closely.

        We do not know the milk levels attained by this product, but I
assume it passes easily.    But this is one product that could potentially
be very dangerous to the infant,  passing into human milk via the  milk
triglyceride, and concentrating in the pediatric liver.  I simply would
never recommend anyone use this product while breastfeeding,    Absolutely
never.



ISOTRETINOIN
Generic  = ISOTRETINOIN
Category = Vitamin A derivative used for acne
EXTREMELY DANGEROUS PRIOR TO AND DURING PREGNANCY.   Secretion into milk
unknown but suspected due to lipid solubility.   Much too dangerous to use
during breastfeeding.  Definitely CONTRAINDICATED.
AHL= >20 hrs.    M/P=    PB= 99.9%
PK = 3.2 hrs.
