Janet--
Two things that you might check for: First, the frenulum, to see if the
tongue is compromised. Second, the palate shape-- if the palate is
particularly high or has a "bubble" spot, that could also account for the
problem. Often babies with these things won't gain well, but they
can.....  I would want to do a digital suck exam to see how it feels on
my finger; sometimes you can glean some clues from this. Let us know if
you screened for the above.
