One grim thought to add: nicotine is a drug, and it may be that the
mother has also used other, illicit, drugs, and is not admitting to it.
Does she want to breastfeed? How does she feel about the use of
Nutramigen over her own milk? Does it bother her, or does she seem
relieved? You may not be able to find out if she has used other
substances, but if she is at all disgruntled about the milk, it could be
tested to ascertain if there are any compromising substances in it. This
is one of those truly touchy situations where some wariness is justified
but outright suspiciousness needs to be capped. Good luck sorting it out.
