Thanks Maryelle, Lisa M and Cathy G. for your helpful feedback. The mother
felt extremely supported to know I'd posted her case on Lactnet.
An update: After a thorough GI workup and eight days on Alimentum, baby was
negative today for blood in the stool and is back to breast (Pedi GI calls it
a "breast milk challenge"). Mother meantime has severely modified her diet
and joyfully told me she's lost three pounds (is considerably overweight).
It's an ill wind ......
Lisa, I was so busy musing over the blood that I completely overlooked the
connection between the greenish stools, the gas and the excellent weight
gain. Thanks for jogging my mind. Turns out this baby did nurse for short
periods -less than ten minutes on each breast - da dum ! -so once mother has
built up supply again - didn't do well with pumping - she will keep baby
longer on the first breast.
My hunch is that given her family history, this is a milk allergy. Let's hope
that restricting dairy will do the trick.
I'm seeing them again tomorrow. Will keep you posted.
BTW - the thrush in baby's mouth has cleared. Mother was amazed at the
improvement when she followed the care plan I gave her (based on info in the
LC Series unit on Candidiasis). She'd been using nystatin drops but before,
instead of after, each feeding. The only instruction she'd been given was
"apply 4 x daily". I really like this mother. She's spunky. She called the
pharmacy and the ped. to tell them they needed to be more specific with the
instructions.
