I don't see why this info should be inappropriate to post, as we all need
it. If Egnell also has props, I would appreciate someone telling us.

Medela makes large, hand-blown glass flanges that cost about $40 a piece
and fit into existing flanges. They used to have two sizes, but combined
them to one, which is very very large, and sometimes too large for the
women who just barely don't fit the regular. Measure the diameter of
mom's nipples, then call Medela.
OR, Bailey Medical Supply -- Nurture III/Double Up --- used to have a
flange model that was larger than the current flanges. While not as large
as the Medela glass flange, it is larger than the normal Medela flange and
may be just the ticket for some moms. Again, measure the mom's nipple
diameter, then call Bailey to find out if their old flange (which you can
still obtain by request) is large enough for these nipples.
