In my experience, the best thing to do (usually) is to pinch behind the
nipple to make it pop out, and then the baby can latch.  One mom I worked
with had nipples that turned all the way inside out, and the baby latched
after reaching in and pulling out the nipple.  Laid back position often is
useful, partly because Baby is less stressed, partly because Mom has a free
hand to pinch behind her nipple without also needing to support her breast.
