Small babies: breastfeeding immediately before frenotomy for pain reduction, mother is with baby during procedure, can support the baby, holding baby's hands, the clipping is done without local anesthesia and it takes 1 second, immediately afterwards breastfeeding for reducing pain.

The pain is similar to an injection or a bite into the cheek, very short. Babies take it well, they cry because somebody holds the head, some stop crying when being lifted again, some after a few minutes.

In the first 24h parents have paracetamol at hand, usually don't use it. Lots of skin contact and body contact afterwards.

The disadvantage of local anesthesia with small babies: if it is an injection, in the same time the clipping is done. If it is a gel, the taste is very bad, you can't tell the baby to spit it out, and this can interfere with breastfeeding, especially directly afterwards.


Babies after 6 months and toddlers: some local anesthesia gel is put on the frenulum, procedure in the same way.

Scissors frenotomy is done without general anesthesia in ALL ages. General anesthesia is not required for such a small intervention and its risks outweigh any benefit. For the procedure itself, the main disadvantage of general anesthesia is that the tongue does not have its natural tonus and its very difficult to cut appropriately.
