We've been using EPIC for many years in our outpatient clinics. We have
scheduled lactation appointments and chart in the progress notes of that
appointment., with smart phrases that we've created.  When I see someone
who does not have an appointment to see me (maybe was scheduled to see a
pediatrician who asks me to "drop in"), I can go under the "encounter" tab
near top of screen, then click on "clinical documentation" and create a
note enter vital signs & weight, etc.
I don't know if the set-up is similar in other systems' EPIC charting.

My apologies for the EPIC jargon to those who don't use it.

Sorry to hear that the MDs find it onerous to see your notes; that alone
says something.
