Unfortunately I think that a lot of what happens with moms and babies in
our hospital is arbitrary. I have really enjoyed reading what you have been
writing about hypoglycemia. The other day we had a baby in the nursery
while her mother was in recovery after a c-section. The baby was 3.900 so
we had to check dex. The mother did not have any sort of diabetes. The
baby's dex at 1 hour was 47. The baby was calm and asymptomatic. The nurse
in charge insisted on calling the doctor to report that the baby's blood
sugar was low (47 at 1 hour!!!) and the doctor immediately said to feed the
baby. Colostrum wasn't available but you know, who cares, right? I

But it got worse! That same day, we had a 37 weeker, 2.200 kilos, The
baby's sugar levels were always above 50 and body temp also good. The baby
wasn't being monitored. We brought the baby to the mom and checked that she
was being fed (unfortunately at that point the mother was unable to express
colostrum - so was I - and the baby absolutely wouldn't latch) ' mom was
giving a bottle with the amount of formula that she had been advised to
give. Anyways, this same doctor, a senior doctor, had a fit that such a
small baby was with her mother and not with us in the nursery. I was sent
to bring the baby. I explained to the mother that the doctor wants such
small babies in the nursery because they can't always mantain proper body
temperature or sugar levels. The mom said, but you've been checking her and
everything's fine. I had to agree. I left that baby with her and got hell
instead. I can't imagine how this baby could be better off in the nursery
in a plastic box with us than in her mother's arms - which is where she was
all day. Everyone kept saying - well what's the protocal?? At what weight
can we let the baby out of the nursery?? Apparently there is no protocal -
which makes sense, since every case should be judged individually. So what
if she is 2.200 kilos or 2.400 or 5 kilos for that matter?? I left that day
so upset and had to go back that night. Night shift I discovered that the
nurses that came after me had ignored the advice of the doctor and mother
and baby had stayed happily together throughout the day and mom had even
managed to express a great amount of colostum.

Anyways, thanks for letting me rant, by the way, we do not do random dex
checks after c-section, or ever, only for babies at risk.
