I would bet you that this baby is just not getting anything at the breast, and 2 oz every 3 hours is just enough to maintain his weight, but not to gain.

In my opinion, baby should have been offered as much as he would eat of pumped milk/formula as soon as weight loss was noted to be >10%  AND there was no stool.  Continuing with daily naked weights until he was steadily gaining well.

 Is it possible this mothers' nipples have grown larger with each child?  I see that sometimes as a reason everyone is surprised when bf does not go well.
