You may be describing subclinical mastitis. Symptoms include sharp,
needling pain and/or burning sensation in the breast (Fernandez et al. Benef
Microb 2014;5:169-183). Mothers may describe the feeling as abrasions from
cut glass. It can be misdiagnosed as candidiasis and incorrect anti-fungal
treatment may delay or exacerbate it. Bacteria responsible for subacute
mastitis do not produce toxins (like S aureus does in acute mastitis) but
form thick biofilms in ducts. S epidermidis and coagulase negative bacteria
may prevail here. The combination of Strep. Viridans and coagulase-negative
staph form thick biofilms resulting in impedance to milk flow through the
lumen of lactiferous ducts. This pressure on the narrow inflamed lumen
leads to the needle-like pain and burning feeling (Angelopoulou et al.
Medical Microbiol Immunol 2018; doi: 10.1007/s00430-017-0532-z and Fernandez
et al. Bene Microb 2014;5:169-183). A culture and sensitivity test of the
milk might reveal these types of bacteria. Probiotic therapy has been
suggested to alleviate the bacterial dysbiosis using specific types of
probiotics-- Lactobacillus fermentum CECT5716. Products with this type of
probiotic are commercially available in many countries and of course on the
internet, such as Lactanza (Spain), Qiara (Australia), Target B2 and
Elactia (United States) (although I am not sure if Elactia contains
Lactobacillus
fermentum CECT5716 or just Lactobacillus fermentum). These might be helpful
for your situation.
