Hi!

Love to read all the input that you share, with soooo many years of
experience... In my growing experience (IBCLC since 2005) I see the same
thing you mention: every case is different, every dyad requires their own
position and adjustments, which then go on to change as the breasts go
beyond the initial fullness and as the baby grows and becomes more
practiced. Such a dynamic interaction!!

I think some people in their need to a) help others and b) develop a method
that will give them renown and/or income, try to protocolize things that
are un-protocolizable (if that word even exists). I get it, I have also
felt the need to understand the exact steps to a perfect latch and to
somehow get this information out to parents and HCPs in a way that works
for everyone, that everyone can apply. Sadly, it is impossible due to human
variability. Certainly we can get the general idea out there... but beyond
this, it´s just mom, baby and someone with patience and experience.

About the medicalization of breastfeeding... what a difficult topic!! As a
MD IBCLC I feel fortunate that I can diagnose and prescribe, I feel it
saves families time spent on searching for another provider, and it also
means I don´t have to convince a doctor to prescribe what I deem to be
necessary. However, I also feel bogged down sometimes by my medical
mentality, by thinking that many things are pathologies when they are not,
by believing that I know more than my patients about themselves. As I
become older in this profession, these things happen less and less as I
grow farther from being "a doctor" and closer to being "a breastfeeding
supporter". I´ve learned to listen and be silent, I feel freed by having
discovered that most mothers know what is wrong and give me pointers on how
to help them, if I just create the safe space for them to do so. I´ve
learned to look at babies, really look at them, their faces, their
expressions, the way they communicate their needs in a thousand subtle (and
not so subtle) ways.

Anyway, sorry about the ramble, I just feel very grateful to work in this
field and to have learned that the only valid method is the caring
interaction between a well trained provider and her client/s.
