First and foremost, as  you did, refer to pediatrician for evaluation.
Next, I would vote for oversupply, overactive letdown that has baby
"scared" of the flow.
Try to entice baby when sleepy, refer mom to the positioning etc strategies
and then possibly full drainage block feeding. As always, with any change
in baby feeding or bf management, monitor output and weight.
