HMBANA has a wonderful resource called “Best Practices for Expressing, Storing, and Handling Human Milk: In hospitals, homes and child care settings.” A worthwhile purchase for anyone in those settings. Available at https://www.hmbana.org/publications.
There is a section “Human Milk Error: when the wrong milk is fed to the wrong baby” with information for hospitals, day care centers and parents. The section also includes risk factors, sample policies and flow charts. Too much to include in this email. Most important is for the institution to have a clear policy, chain of responsibility for executing it and clear information for parents and risks and what the policy is.
