I also am at a small community hospital.  I suggest you reach out to the Obstetrics/Women's Health, Pediatricians and Family Practice in your area to let them know you offer this service.  The can send you referrals. 
Contact the LC @ neighboring hospitals, birth centers or tertiary care hospital where the Moms from your area may delivered. They could certainly share your information.  I think it's important to work together in the best interest of the mother/family.  If a Mom lives closer to another facility/ LC, I definitely suggest they go there. It's more than a notion sometimes to pack up a newborn and drive an hour for a consult.   
If your hospital promotes education classes, support groups and services on Social Media or Local Newspaper they may be able to provide your contact information.
