This baby and mother had a very difficult time, indeed. Remember the baby
is only 3 wks corrected age now.
I think I would try to help mother simplify her life. Some of the things I
think would help would be
-wearing the baby in a sling or wrap, kangaroo mother care. this can help
with milk production and baby weight gain.
-keeping the breastfeeds short, maybe 10 minutes each side with
compressions, and then topping up with expressed milk
-possibly skipping the direct bf every other feeding, instead expressing
and bottlefeeding the every other feed times (this to conserve energy and
time for both mother and baby.)
-calling in all resources so that mom can rent a double electric hospital
grade pump; I cannot imagine doing this with a manual pump
-as much help with the other children, household chores, food prep etc as
possible
-galactagogues (our doctors use reglan and most mothers feel it does help
their supply; you may have access to other meds with better side effect
profile)
-I personally would probably leave aside the body work or tongue tie evals
for now
