Unless joint commission has very recently changed or updated PC-05 it read that if a mother chooses not to breastfeed then a 'discussion' had to take place with the mother and this documented.
So, if the provider discusses with the mother the risk of formula feeding then in the providerd notes he can document that he has had x conversation with the patient. That should be sufficent for Joint Commission.
But, if they have changed the wording of this could you please send me a private message with a direct link to which states the mother has to sign a document - because we would need to be changing our practice.
