I have a client who is on her 4th day post D and C (at 22weeks gestation for nonviable fetus) who is engorged.
Her current care plan includes :
Snug fitting sports bra,
Cabbage and Cold Compresses ,
Ibubrufen 4oomgs q6hrly,
Sage tincture q8 hrly,
Peppermint.
Sudafed,
Vitamin B6 100mgs once daily(this is what she is comfortable with).
I would appreciate any advice as this am the engorgement seems more pronounced.
She does not have a fever and there is no apparent redness.
My client would prefer not to express to comfort as she does not wish to stimulate any production.
I have read about Caster oil compresses and am wondering if anyone has tried using this treatment for engorgement and how effective it is. I am reluctant to try as it is a warm compress and may cause milk drainage.
Should I be advising my client to express for comfort under the present circumstances? Is it unavoidable?
Any advise is appreciated,
Marguerite Gilligan,BSN,CLE.
