Hi Brenda,
The dissatisfied NICU mom has done you a great favor by voicing and having
documented her non-optimal experience regarding feeding plan inconsistency
and therefore negatively effecting the patient/customer satisfaction score
for the NICU. In good hospitals managers are held firmly feet-to-the-fire
responsible for patient satisfaction scores, and for the plans and
initiatives to improve scores. So, you have an opportunity to jump on.
I much like Beverly's suggestion to work with the policy writing team, and
to attend rounds as often as possible. As you note; NICU babies have
complicated problems and issues and they may stay in NICU a long time. This may
be precisely why your valuable input may be overridden in just a few days
after given, or maybe in just a few hours, ...so many dynamics going
on/frequent changes in baby condition/status/needs = feeding plan revisions needed
frequently.
Additional improvement processes that have worked in my experience
include: mandatory NICU staff education (ie: during NICU staff orientation,
regular inservice ed, and yearly competency reviews/testing) And having LCs who
had a specific amount of time devoted to NICU each day. This progressed to
LCs devoted to working solely in the NICU.
Becoming more at ease in the NICU will come with more experience there.
Daily/routinely make rounds, or visit a mom/moms pumping, whatever amount of
time you have to devote there.
Regarding all of the above: If you can make even just baby steps in the
right direction - bravo!
