I have a mom that has been exclusively breastfeeding for the past 6.5 months.
Baby born 06/05/15. Baby is going to breast 6-10 times in 24 hours for about 10 minutes each feed. He is fussy and not content at the breast (recent behavior change).
At 4 months post-partum, she had 3 menstrual cycles in that month, then started her period 12/21/15.
3 weeks ago, she felt like she woke up and had "no breastmilk." Baby's stooling has changed, he now poops about every 2-3 days. He's very fussy at the breast, goes on, pulls off, goes on, cries for most of a breastfeed. Mom has been pumping 4 times daily over the past two weeks, she's getting 5.5-7 oz total from those 4 pumpings.
Test-weights of the baby showed:
12/17/15: 0.7 oz from right breast after 10 minutes
12/29/15: 1.1 oz from both breasts after 8 minutes
Infant weight gain over the past 19 days has been 0.463 oz/day.
Mom started Go-Lacta last week, hasn't noticed a change in her supply. She had her thyroid labs done last week and all were WNL. She is having her mid cycle hormones tested on 01/11/2016.
I suggested she ask for luteal phase defect evaluation, pre/post prolactin levels with a feed, and discuss pituitary fxn with her midwife. Is there anything I'm missing? Has anyone seen this before?
She is very weepy and had intended to breastfeed this child well past age one.
I'm unsure how to counsel her at this point.
Any insight would be greatly appreciated.
