 Debi, I read the document but our problem is mothers who change on day 1 from breastfeeding to supplementing or full formula. I don't see where the new wording addresses this. I've asked many times to change our forms from the current "breast, bottle, or both" boxes to "exclusive breastfeeding only, partial bf, fully formula feeding." I think this lends itself more to the appropriate discussion with the mother. Also the new wording assumes, expects that hosp staff are discussing and recommending excl bf, not just asking mothers' their choice on admission. Our rates of excl bf have been running between 15 and 20% (rural Mississippi USA). Our exclusion forms do not have a box for maternal illness, only ICU admission. Sometimes we have mothers so conked out on magnesium sulfate, pain meds, and perhaps receiving blood transfusions but on the OB ward. Also we see a fair amount of insufficient glandular tissue and there is no box for that (reason to supplement). For example I have a mom today on discharge day virtually no milk, hypoplastic breasts, and we are going to use a lactation aid, but there is no exclusion for that?

<< anyone else going crazy with the "wording" of these core measures??? I see that they plan to update us in August 2014. We are in the midst of changing "bottle" to "formula" but our culture has used "bottle" for so long in our speech and documentation.
Curious to know what other hospital's (besides baby friendly ones) statistics are looking like?  >>
