I don't know if the following is useful re BFHI but it seems to be a good model:

In August 2010 the Connecticut Children’s Medical Center, Hartford, CT, USA officially adopted a policy of using human milk as the standard of care - mother's own and/or donor milk - for all babies equal to or less than 1800g or equal to or less than 32 weeks gestation.

Dr Kathleen Marinelli is the Center’s neonatologist and director of Lactation Support Services and also a Fellow of the Academy of Breastfeeding Medicine (FABM).   

In 2011 the NICU increased from 34 to 72 beds.
