Rachel Lynch asks:
I was wondering if anyone has creative solutions for a premature infant (former 26 weeker, birthweight 960 grams) receiving breastmilk, significant reflux with associated bradycardias, and very poor weight gain. Have attempted multiple ways to fortify milk, and have tried unfortified milk, nothing is working. Nursing staff has tried multiple positions to no avail either. I appreciate any input!
I work with a lot of mothers with babies with reflux. When the gut is damaged, as are virtually all premature infants' guts, you need to address the problem from several directions. First, the baby needs an assessment for TT and LT. Also, the baby needs bodywork, as vagal function is likely involved. Finally, mom's diet needs to be addressed--usually dairy and gluten must be eliminated. Slippery elm bark powder added to a little milk before feeds often helps tremendously
