Hi Lisa, My first thought was about the developmental delays. I don't know how significant they are but the child may have growth issues due to the underlying problem, which may not have even been identified. It could be a genetic or metabolic issue. Perhaps the mother (if able) should seek a consult with a developmental pediatrician or peds associated with a children's hospital. There are many possibilities and syndromes and mother will likely get more help and insight if she can do this.
