I'm hoping that someone else has experience with this at their hospital.  We are having an increase in requests for use of informal milk sharing with mom bringing in milk from a friend either for their NICU baby or adopted baby or baby who needs medical supplementation in the first couple of days in the hospital.  We did start using donor milk about 1 year ago but currently the policy is only for babies less than 34 weeks.  These have so far always been moms who are either pumping for baby but having a delayed onset of lactogenesis II or low supply in a longer-term NICU baby.  The Peds groups would like us to draft a waiver for parents to sign that lists what the risks of unscreened donor milk are before letting them use it.  I would also like to add risks of formula, of course.

Does anyone already have a form that their institution uses?  I know we'll have to work with legal/risk management but thought I'd gather ideas before drafting something.
