Hi Angie ~

Congrats to this new MOT! However, this doesn't sound as if it's a "twin
BF" issue -- sounds more of an individual one or both babies issue --
unless she's BF both at the same time, which takes some coordination and
makes it more difficult to monitor each baby to determine whether one/both
contribute to the problem and to deal with nipple/breast issues. So if BF
both simultaneously, I'd suggest she BF one at a time for now, so she can
pay close attention to the latch and suckling behaviors of each -- and
intervene more easily.

I'm in doubt that both baby A and B are latching all that well. Has each
baby been thoroughly assessed re: oral structures - tongue, palate, lips,
etc? When is the last time an IBCLC or other experienced BF worker actually
observed a BF? What does her nipple look like immediately after baby A
detaches and what does the nipple look like immediately after baby B
detaches? Is there creasing, flattening, etc? Is there skin damage? (If so,
what kind and where?)

Is there unresolved engorgement?

Do babies receive any bottle-feeds - even if EBM is in the bottles? If so,
feeding technique and type of teat can make a difference when at breast.

What does the mother think may be contributing to the pain?
