A 32 year old mom who has a history of PCOS for several years conceived through IVF and her birth went horribly wrong and she ended up with 2 blood transfusions and was in Intensive care unit for several days before discharge. Baby was FTT before mother approached for help. Baby is doing well with formula and some amount of expressed milk. Mother manages only 20 to 30 ml at each expression and manages 200 ml in 24 hours with 8 expressions with a hospital grade pump. The amount increases from 60 ml in 24 hours to 200 ml with breastfeeding management over several weeks. Mother and family are at the end of the rope. Baby suckles is strong and rhythmic. Mother feeds expressed milk + formula using a homemade SNS.

She is on 30 mg domperidone three times a day along with other galactogogues. She has very little help at home so unable to manage more than 8 expressions. What else can I do to help her increase her milk production?
