Tough question...
I started my (very informal at that time) lactation work 30 years ago in 
the country that did not have lactation consultants at all, where kids 
were routinely supplemented in the nurseries and where the only formula 
available was a powered bovine milk with crystals of table sugar in 
it... And did I say that all baby's ailments were blamed on the "bad 
breast milk" so the recommendation to stop breastfeeding was a standard 
routine.
