I saw a 3 month old baby who has been on zantac for 4 weeks without any
relief from what appears to be pain in the upper GI while nursing. This
baby was healthy weight. Is very calm in between feedings. Sleeps 7 hours
at night without waking up. And nurses for only 1-2 minutes on each side as
she is pulling off and on crying. She doesn't seem stressed until she pops
off... cries... goes back to nurse with audible swallows and then pops off
crying again. There is no gulping but you hear normal swallow sounds and
then there is some air or I don't know what the word is... movement that
you hear almost come from the stomach to the esophagus. She doesn't spit
up. She does burp and she seems perfectly content after she has had
something in her tummy and done nursing however short of time. Mom said
that she had "tender" nipples for first 6 weeks but claims she had no
"real" problems... then all of a sudden 4 weeks ago the baby started to
show signs of reflux... popping off and on with crying. Mom has now
switched to previsid. Any other thougths that would help this mom I would
really appreciate it. Her other babies who she did not nurse for this long
with also had acid reflux and she said they were spiters and grew out of it
by 6 months when they were sitting up more. Mom feels this baby does not
spit up at all and appears to be in pain during feeds and I agree. Mom has
also done the milk elimination diet for the last 4 weeks before coming to
see me, also without any changes in baby. Any thoughts or suggestions would
be very helpful. Thank you.
