It's the opposite of high arched palate, the tongue strongly spread the
palate. Sometimes occurs when the tongue is very large, as in
macroglossia (a tongue that is so large it does not fit in the closed
mouth).
The big issue is helping the baby get sufficient breast in the mouth
that the large tongue can do it's job. Rebecca Glover's technique of
tilting the nipple away a bit, then pressing through the breast to slip
a little more against the tongue as baby latches can help. Baby may
need supportive techniques, more time to feed, or even supplementation,
preferably with expressed milk.
