This has been discussed extensively in the archives.  It is the lack of protein that causes poor growth, not fat.  See also www.cpqcc.org, click on quality improvement, then toolkits, then improving NICU nutrition toolkit for evidence-based recommendations.  If your NICU is a part of VON, they have a toolkit with the same recommendations.
