Oh Jane,
If only your post had come in sooner, I could have saved everyone's eyes
by not sending mine! Yes, "initiation and volume-increasing" vs.
"maintenance". Still even maintenance level pumps often fail when they take over too
much of the maintenance work ( babies who have been trained to, or
self-select not to eat for 10-12 hours every night at young ages, whose moms work 5
days a week and there are not enough "milk removals" as Nancy M has
written, etc...)
