Hello everyone,

ptp: Mom is 8 days pp. Baby has not latched for any significant amount of 
time. Mom has short nipples and baby has a tongue tie. I introduced a 
nipple shield during my visit, but baby didn't latch. Tongue tie being 
addressed this week. Mom is pumping and has a great milk supply. She has cracked nipples. Possibly from baby, although I suspect it was from 
turning the pump up too high (we worked on that and flange fit 
together).


Here's the interesting part: today (day after visit - love it when they leave 
out pertinent/related info like this), she mentioned that she is 
experiencing intermittent breast pain. She said it feels like stabbing 
right behind her nipples and that she has a rash on her arm. The pain 
lasts 10-15 minutes, so I don't think it's let down. She thinks 
ibuprofen is helping, but maybe the pain subsides on its own? During her 3rd trimester, she also had the same intermittent breast pain and a 
severe, itchy rash all over her body (OB ruled out choleostasis). She 
took antihistamines and they took 1 week to work. The current rash is 
occasionally itchy and is slightly raised. She said it gets worse in the shower. I didn't notice it during my 
visit and she was topless while we tried to get baby latched. She had 
abx in labor for GBS. I recommended she start high doses of probiotics 
when we met. I don't suspect thrush. Has anyone seen something similar?

She's
 the 3rd client in the last 6 months that has had random 
itching/rash/hives postpartum and I haven't been able to find a cause.
