Could it possibly be OAMER? and a bottle flow that is not given in an upright position so as to remove gravity from the equation?? Could he possibly be protecting his airway? Does he do the same thing in a relatively "laid-back" position so that milk is flowing against gravity? 

Also,  a great new position called "Koala position" was pictured on a poster at ILCA that might also help. Mom sits upright with baby upright, straddling one of mom's legs and steadied with a single hand to the back. This frees mom's other hand to support the breast from the outerside and below, at a 90 degree angle from the body, so she can touch the nipple to the upper lip to stimulate a wide open mouth and continue helping the baby latch in a position where the direction of the milk is 90 degrees outward, not downward toward the baby's airway.

Just a few ideas that came to mind with that description of the baby's reactions.
