What you found, about the high caloric content of the milk of babies 
who were not gaining well, is extremely important, I think!  It makes 
absolute sense.  When the breasts are emptier, then the fat content 
of the milk is higher.  And when production is high then the water 
and lactose content are also higher, which would (presumably) lead to 
lower calories per millilitre.  From working with failure to thrive 
babies,  I know that - whatever the original cause of the problem - 
by the time the mom consults the LC the result is usually severely 
depressed production by mom, resulting in insufficient intake by the 
baby, quite literally "not-enough-milk" (low quantity), and that the 
small quantity that can be expressed after the baby has been hanging 
out (not swallowing) at the breast for hours and hours is usually 
comprised ot thick, creamy drops (high quality).  Please do think 
about submitting those case studies for publishing - this is an 
important part of the FTT cascade for which we need more "evidence"!

IN testing mom's milk of babies not growing well (of all ages), I 
found that the calorie count was not insufficient, in fact was high 
27 and up! I need to submit some case studies for publishing. Anyhow, 
the issues were insufficient intake, not quality of intake. .....
