Rebecca,
It sounds like there could possibly be some neuralgia, nerve compression, neuromuscular impingements, etc going on.  All of which can cause symptoms that you mentioned, including pain and decreased milk flow.  Has mom had any thoracic, brachial plexus, etc related injuries/surgeries or pain prior to delivery?  When working with similar cases, I refer to specific breast-friendly physical therapists, body workers, acupuncturists, etc in cases where nerve or structural issues are the culprit.
