(With apologies for strange formatting.)  In Canada, the BCC has integrated hospital and community BFI steps into a single document.  The 10th step states:

Provide a seamless transition between the services provided by the hospital, community health services and peer support programs. Apply principles of Primary Health Care and Population Health to support the continuum of care and implement strategies that affect the broad determinants that will improve breastfeeding outcomes. 
Which includes:
Work collaboratively with each other and community breastfeeding supports to facilitate a smooth transition from hospital to home. 
Ensure that families receive sufficient information to identify and access available community resources in order to promote continuity of care and support for breastfeeding families. 
Work collaboratively with broad community partners to protect, promote and support breastfeeding as the cultural norm. 
Provide ongoing anticipatory guidance and support for families on continued breastfeeding. 
Inform mothers about their right to have accommodations in the workplace that support and sustain breastfeeding. 
Inform mothers of their right to breastfeed in public spaces. 

That is A LOT more than just "handing mothers a list of resources". Of course, "you can lead a horse to water..." and getting mothers to actually contact those community resources is a challenge in my region.
