So far this is what has happened.  Sore has not yet formed a blister, (takes about 4 days for this to happen) so I don't know how easy it would be to culture it.  So far it looks more like a cold sore than a fissure or impetigo (but great things to consider).  It is about 1/3 the distance along the lower lip and at the boarder between the lip and facial skin. The mother spoke to a friend who happens to be a GP and has been in this situation herself, though with a much younger baby.  The friend did continue to nurse and did develop some small lesions on her breast that have never come back in the 5 years that have passed.  Friend prescribed acyclovir for the child.

I can keep you posted on how things go.
