Hi Jessica,
seems like you are looking for a 'Breastfeeding Assessment Checklist' to
facilitate consistency and continuity of care....right?
Something, for example, that would include pertinent, concise, basic
assessment of mom, breast, baby, oral, latch, feeding, output, ....correct?
If so, try an online search for 'Breastfeeding Assessment Checklist' or
'Breastfeeding Evaluation Checklist' and call larger hospitals to see if they
would share their version with you.
Also wondering: you might additionally be needing 'breastfeeding care
plans' to go along with the checklist, which include listed treatment options
and you checkmark the ones appropriate for the situation.
