Liane, this honestly seems to me to be a case of a baby who is not getting
enough milk. The problem is not that he's not pooping, the problem is that
he's not getting enough milk to make him poop!! Either the mother is not
producing enough or the baby is not transferring the milk (hard to tell from
what you are saying - but the before and after weight of 2 ounces is not
enough, for one thing, and this baby is way below his birth weight at 18
days). I would be significantly concerned.
You mentioned that mom is pumping and supplementing with small amounts of
her milk. Is that because she's only getting small amounts? If so, milk
production may be the issue, and I'd take steps to increase it - more
pumping, herbs, domperidone. If she has more, I'd strongly encourage her to
give that baby everything she has. But given how low his weight is at this
point, I think supplementation with donor milk or formula is likely to be
necessary.
Maybe there is a medical issue, but I'd want to be providing this baby with
more milk first to see if we can rule that out. Give him plenty of
supplement and see if things improve.
