The pictures do the best job of discriminating high from low scorers on
the exam, in other words they really do test your expertise. May I
recommend getting all the books with lots of photos - the Breastfeeding
Atlas, Linda Smith's Exam Review, etc. And follow around all those
experienced IBCLCs all you can to watch them work with moms.
