My experience over many years has been that in cases of severe engorgement,
"pumping out" once, usually after the application of cold/cabbage, has been
enough to get things under control as long as the mom thereafter nurses on
cue, baby has good latch, etc. It's kind of like hitting the reset button
and also gives the mom an immediate feeling of relief and control over the
situation. There are those few moms with intractable engorgement that
require additional measures.
