One of my clients gave me a survey that she received in the mail from the National Institute for Infant Nutrition which of course had no address or phone number.  the enclosed postage paid envelope has the return address as PO Box 11 Woodworth, WI.  53194-9901

Directly addresses the mother, by name and is  signed by Janet M Jones, Research Director? - that's it.

Opening paragraph; "The National Institute of Infant Nutrition conducts monthly surveys to understand nutrition for infants.  This research has been on-going for fifty years and gathers information about the breast-feeding and formula-feeding habits of American households, based on the age of their babies."

It is very detailed, four pages long and asks all about formula - brand, where they got it, samples, baby clubs etc.  It even has colored pictures of each "Family of formulas" : Enfamil (13 kinds) Good Start (7 kinds) Parent's Choice (16 kinds) and Similac (18 kinds).

If you exclusively breastfed from the start, you only have 2 more questions to answer, the second is "Did you receive any formula samples, before during or after birth.  The third question, "How many times your baby fed yesterday and average number of ounces???"  Then "What is the zip code of your baby's doctor's office?"

Yet another disguise and tool of the formula industry!  Hopefully, like most surveys, many will be thrown out!
