The biggest reason we do not see resolution is because the GI has no idea how to treat food allergies. Three days off dairy--even if she was really off--is ridiculous. I tell moms they may need 5-6 weeks.
And I would be very concerned about gluten here. At the very least, you need a history from mom and dad that give clues as to the most likely culprits. I take a very extensive history on mom's diet and nutritional profile and any look for any indications of maternal gut damage. You cannot just eliminate foods--you have to heal the mother to heal the baby. I start mom on gut healing as soon as she begins food elimination.
Mom needs to know that she is not getting permission to nurse her baby--it is her right to find a solution that allows her to bf. I consider it the doc's obligation to help her solve her problem, not her obligation to give up.
And I would not assume there are not other factors--at the very least this baby needs structural work.
